This form is to be used by a person or group of people who have feedback that they would like to pass onto Upper Hunter Homeless in writing. Providing the information below and confirming your contact details (Optional) will assist staff at Upper Hunter Homeless to address or consider your feedback in a timely manner.

Alternatively, you can call (02) 6542 5051 and request to speak to a Management Representative of Upper Hunter Homeless who will meet or speak with you for a confidential recount of your complaint or feedback.
